SEN Transition Support Officer

Seven Resourcing are looking for a temporary SEN Transition Support Officer to work in Shrewsbury, Shropshire. This position is initially for a period of 3 months and is paying £16.53 – £20.88 per hour (PAYE and Umbrella payment options are available).

The role:

The post holder is required to support the Special Educational Needs Team Manager in the efficient and effective delivery of the statutory functions of the Local Authority in the efficient and effective delivery of the statutory functions of the Local Authority.

The main purpose of the post is to enable young people with learning difficulties & disabilities to achieve their full potential at key transition points, and in so doing, make suitable, well informed decisions about next steps in education, training or employment.

Key Responsibilities:

      Responsible to the SEN Team Manager who is in turn responsible to the Special Educational Needs Service Lead who is directly responsible for the post holder’s health and safety, training and development.

Responsible for caseload management of children and young people 14 – 25 years who are transitioning into post 16 provision. This will involve coordinating multi agency involvement across education, health and social care to facilitate positive outcomes across transition between children’s and adult services.

Responsible for monitoring of data relating to young people with an EHCP who are Not in Education Employment or Training (NEET).

To support SEN Officers with student re-engagement and young people transitioning into post 16 educational settings.

Support the SEN Team Manager with other key transition points such as End of Key Stages in education.

To write and amend Education Health and Care plans and manage casework throughout the Education, Health and Care needs assessment (EHCNA) from first referral to completion and review, ensuring a person centred approach and functions are carried out in line with LA policies and SEN legislation to the standards and timescales expected in the Code of Practice 0-25.

Responsibility for managing SEN transition casework and monitoring the “Preparation for Adulthood” transition (14-25 yrs) process for young people with EHCP’s and/or high needs funding ensuring functions are carried out in line with LA policies and SEN legislation to the standards and timescales expected in the Code of Practice.

Monitor sufficiency of provision, based on caseload need, make recommendations to the line manager regarding how to raise local capacity in provision with appropriate supporting evidence.

To provide support and challenge (where appropriate) to settings including Head Teachers, Principals and agencies.

To support the annual review process for children and young people 14-25 in educational settings who would require provision through an EHCP or high needs funding.

To promote a multi-agency partnership approach between children’s and adult services in coordinating a person-centred transition for young people with Education, Health & Care plans and/or high needs funding.

To work with the arbitration and mediation services to resolve disputes, this may include liaison with SC legal services, Information Advice and Support Service (IASS) and formal mediation services (but is not limited to these services).

  • Qualifications: N/A.
  • Experience: 1+ year of experience within a similar.
  • Compliance: Proof of ability to work and live in the UK. Current DBS and references covering the last 3 years.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37 hours p/w.
