Adult Assessment Social Worker

Seven Social Care are seeking a dedicated and compassionate Adult Assessment Social Worker to service our diverse community in Derbyshire.

Job Description:

As an Adult Assessment Social Worker, your primary role will be to provide support, protection and social care services to vulnerable adults.

Responsibilities:

1. Provide comprehensive assessments of adult social care needs, considering physical, emotional, social, and welfare issues.

2. Regularly evaluate and reassess the individual’s needs, making adjustments to care plans as necessary.

3. Formulate, implement and review support plans and suggest interventions that promote independence and better living.

4. Work closely with other professionals such as nurses, doctors, police, and probation officers to ensure the provision of a coordinated and holistic approach.

5. Communicate effectively with clients, their families, and other organisations to provide accurate information, advice, and support.

6. Act as an advocate for the clients and challenge decisions when necessary to ensure their needs are met.

7. Ensure the safeguarding of adults to protect them from harm and abuse, and report any concerns in accordance with established protocols.

8. Complete accurate and concise records, reports, and other documentation related to the clients’ cases.

Qualifications:

1. Degree in Social Work.

2. Registered with the Social Work England as a qualified social worker.

3. Proven experience in a similar role, preferably within a local authority setting.

4. Knowledge and understanding of the relevant legislation, statutory guidance and best practice in adult social care.

5. Excellent communication, interpersonal and organisational skills.

6. Ability to handle complex and sensitive situations, demonstrating empathy and respect.

7. A valid driving license is desirable as the role may involve travelling across Derbyshire to visit clients.
