Adult Community Care Officer

Seven Resourcing are currently seeking an Adult Community Care Officer to work within Nottinghamshire County Council. This is a full time role, paying £16 p/h. 

Full time role and driving licence is required.

Hybrid roles, no particular office-base but CCO will be in based North, Mid and South Notts.

Predominantly at home.

JOB PURPOSE

To be responsible for both the assessment of individual needs and the initiation and coordination of a range of outcomes to meet them, promoting the principles of choice and control, personalisation and self-directed support to ensure that service users can assess and manage their own needs, risks and uncertainties.

The posts address will be County Hall but there will be very little requirement to work there. Instead post holders will work in a hybrid way. Each post holder is likely to focus on either the North, Mid or South of Notts depending where they live and will undertake reviews mostly in that area but may be asked to go to other parts of the county. Travel expenses will be paid. They will be able to “hot desk” at county offices and may be required to come in for meetings.

Candidates need to have some Social Care experience. It would be preferable that they have some experience of support planning for people with social care needs. Ability to build relationships quickly, to work with people from a variety of backgrounds and to partnership work with family and other professionals. Ability to have difficult conversations and assess risk. Understanding of the Care Act and Mental Capacity Act. Desirable previous experience of using Mosaic. Training and support can be given.

The job will be based in a new team and will be undertaking the targeted review of existing support by focussing on peoples strengths. The team is county wide, but team members will be able to specify which part of the county they wish to work in and whilst we would not always be able to guarantee this, we would make every effort to accommodate this. Candidates will be supported by a manager and Advanced Social Work Practitioner and will link closely with community team and providers to consider creative ways for people to be as independent as possible.

EXPERIENCE

At least two years experience of care work gained through paid employment or extensive relevant personal experience or voluntary work. Experience of operating as part of a team and assisting others in their work. Experience of independently managing and prioritising demands and tasks to meet objectives.
