Adult Social Care – Social Worker

Seven Social Care is looking for a qualified  Social Worker in Walsall Council

The role:

1. To undertake assessments and practice in a way that promotes self-determination, independence and empowerment, that builds upon individual and community resilience and supports community integration.

2. To work with service users, their carers and advocates (where appropriate), in the assessment of strengths, individual needs with a view to achieving independence and positive outcomes

3. To ensure effective and efficient provision of appropriate Social Work services 4. To act as the primary point of contact for care arrangements in respect of designated service users, working in accordance with agreed joint policies and procedures.

5. To monitor and evaluate the aims and objectives of proposed interventions
