Seven Social Care is looking for a Adult Social Worker for London Borough of Barnet

The role:

The qualified experienced social worker will be working with individuals who are inpatient on a long stay mental health ward called Ken Porter ward at Springwell unit Barnet hospital.

The Social worker will need to have experience of working with people with a mental health issue and possibly other disabilities.

The person will work with the multidisciplinary team, including ward staff, health and social care professionals to complete Care Act Assessment for individual so that they can be discharged from the unit with appropriate support in community based health/social care provisions.

The person should be confident in using information systems such as RIO and Mosaic and be able to set own priorities
