Adults Senior Practitioner

Seven Health & Social Care are seeking a skilled and compassionate Adult Senior Practitioner to join our dynamic team in Derbyshire, UK. The ideal candidate will have a robust understanding of social work and considerable experience in working with older adults. Responsibilities:

1.  Carry out comprehensive assessments of individuals’ needs and risks to determine and implement appropriate support.

2.  Provide expert guidance, supervision, and support to social work staff under your supervision.

3.  Develop and implement care plans that meet the needs of clients while balancing resources.

4.  Advocate for and deliver services that uphold clients’ rights and promote wellbeing.

5.  Facilitate multidisciplinary team meetings and collaborate with healthcare and allied professionals to ensure coordinated care provision.

6.  Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with data protection policies.

7.  Participate in program development and continuous quality improvement activities.

Qualifications:

1.  Degree Level Social Work qualification and registered with Social Work England.

2.  Significant professional experience working with vulnerable adults, especially the elderly.

3.  Demonstrable ability to supervise and support staff members, encouraging their professional development.

4.  Knowledge of the legal framework governing adult social care, including the Care Act and Mental Capacity Act.

5.  Strong problem-solving skills, flexibility, and resilience in dealing with complex and challenging cases.

6.  Excellent interpersonal and communication skills.

7.  Must have authorization to work in the UK.
