Adults Social Worker – Adult Transitions Team

Seven Social Care are currently recruiting for an Adults Social Worker to start a new role on the Transitions Team in Ellesmere Port.

The Adult Transitions Team works with young people from 18 to 25 with disabilities.

Responsibilities:

• Assess the need for social work service including the identification of risk and the need for protection, determine the method of intervention, and decide and advise on the use of appropriate social services and/or other resources.
• Liaise and negotiate with other professionals, statutory and voluntary agencies to ensure that the best possible service is provided for clients.
• Manage and monitor an allocated workload within individual and group priorities and County policies and comply with the statutory obligations of the Group.

Applicant Requirements:

Experience: 2 + years in a similar role/team

Qualifications: Social Work Degree, registered with SWE

Compliance: Enhanced DBS, Right to Work in the UK

Hours: 37 hours a week, Hybrid, Monday to Friday

Travel: Full UK driving license
