Advanced Practitioner/Team Leader

Seven Social Care are looking for an Advanced Practitioner/Team Leader to work with Somerset County Council in the Immediate Care Team. This position is paying up to £44.28 per hour. 

The Role:

The role provides an exciting opportunity to work in a multi-disciplinary setting, with a focus on strengths based practice and promoting independence and recovery.

Key Responsibilities:

• Goal setting, completing Care Act assessments and devising care and support plans, completing reviews, completing Mental Capacity Assessments, chairing Best Interests meetings, crisis intervention, signposting, providing advice and information and working with people with complex social needs.
• There is also a Safeguarding function within the service, which includes completing initial fact finding and assessing and minimising risk

Applicant Requirements:

• Qualifications: Degree approved by Social Work England. 
• Experience: 2+ years working with adults as a qualified Social Worker in the UK.
• Compliance: Up to date SWE registration and DBS.
• Travel: Clean UK driving license with the ability to commute to and from work independently.
• Hours: 37 hours p/w.
