Band 6 Mental Health Practitioner (Social Worker)

Seven Social Care are seeking a devoted and experienced Band 6 Mental Health Practitioner (Social Worker) who thrives in a challenging and dynamic working environment. The successful candidate will be skilled in providing holistic mental health support, with a deep commitment and understanding of mental health care.

Responsibilities:

1. Manage a caseload of service users experiencing a range of mental health issues, providing interventions and support tailored to their individual needs.

2. Conduct comprehensive mental health assessments, developing, implementing, and reviewing care plans in accordance with best practice guidelines.

3. Advocate for service users, ensuring their views are represented and their rights upheld, and provide emotional and practical support to families and carers.

4. Work in collaboration with other health professionals, including doctors, nurses, and psychologists, in a multidisciplinary team setting.

5. Maintain accurate, confidential patient records in accordance with professional and legislative requirements.

6. Develop and maintain strong relationships with community resources and organizations to enhance service delivery.

7. Attend and contribute to regular team meetings to ensure continuous improvement in service delivery.

Requirements:

1. A Degree in Social Work and registered with the UK’s Health and Care Profession’s Council (HCPC).

2. Prior experience working as a Mental Health Social Worker, preferably within a community setting.

3. Sound knowledge and understanding of the Mental Health Act, the Care Act, and other relevant legislation and polices pertinent to the role.

4. Exceptional interpersonal and communication skills, with the ability to build rapport with diverse individuals and groups.

5. Strong problem-solving abilities, with a proactive approach to providing care and support.

6. The ability to work well in a team and also independently, taking responsibility for the management of a complex caseload.

7. Enhance DBS check clearance.
