Care and Support Broker – Lifelong Services / WAMHS

Seven Social Care is looking for a Care and Support Broker, for an exclusive opportunity with West Sussex County Council.

Location is Bridge House, in Worthing.

The role:

The Combined Placement and Sourcing Team (CPST) at West Sussex County Council supports people with adult social care needs to access appropriate care and support services. The CPST works with customers and service providers to identify, source and arrange care services including domiciliary care, supported living, day care and residential based services. CPST is currently expanding to now include customers with learning disabilities and lifelong conditions as well as working age adults with mental health needs.

CPST staff will each have a number of cases where they need to work with customers and their families to identify how their assessed social care needs could be met by services. CPST will also link in with a range of service providers to refer and arrange support for customers ensuring support plans are clear and that services are purchased correctly. Staff are required to be organised and be able to prioritise their case load whilst having great communication skills to work with customers and families, health and social care professionals and providers. Staff need to be committed to a customer centred approach and ideally have some knowledge of the needs of the customer base (learning disability, lifelong conditions, working age mental health) and/or services that the new team will cover.

West Sussex would prefer for candidates to attend Bridge House every day, as they are setting up new service and need staff together for training and building a team. Other working arrangements can be considered for the right candidate.

Applicant Requirements:

  • Experience: Experience in a similar role
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37 hours p/w.
