Community Care Officer Level 2 – Adults

Seven Social Care is looking for a Community Care Officer Level 2 – Adults at Lincolnshire County Council

The role:

Experienced Community care officer to support busy community hospital and transitional care team.

To support timely community hospital discharges and assessment of need for individuals within the community post reablement support.

Position is a hybrid model however will be required to undertake face to face visits both within the hospital care setting, care home and the individuals own home across the east area of Lincolnshire.

Will hold a case load and be responsible for ensuring all required documentation is completed and all electronic records maintained.
