Community Support Worker

Seven Social Care is looking for a Community Support Worker to fill an exclusive opportunity Brent Council.

 We have opportunities available for up to 12 months and the pay is £20.52 per hour.

 The Hiring manager is looking for someone who can work 36 hours a week (various shifts available from Monday to Sunday).

The role:

To work as part of a team providing a high-quality day-care service for people with learning disabilities, physical disabilities, autism, and challenging behaviour which enables service users to live with a maximum of choice and independence, participating in a wide range of community-based activities, designed to meet their individual need.

To be responsible for the wellbeing of service users, giving adequate support to maximise their dignity, independence, privacy, and choice of how they spend their day and to treat all clients, equally and with respect irrespective of gender, race ethnicity or age.

Key Responsibilities:

To work to support service users in a respectful, honest, supportive, and empowering manner, according to the standards lay out by Brent Council and Direct Service

To build links with Care Managers and work jointly with other Direct Service staff to assess service users’ needs.

To work with individual service users to identify life goals and to work with the Team Leaders to ensure service users outcomes are being achieved.

To facilitate the delivery of service user’s Support Plans together with service users and their careers, drawing on resources and advice available within the service unit and the wider environment of Brent.  To act as keyworker ensuring service users wants and needs are incorporated into all planning cycles.

To support service users in planning and organising social, recreational and leisure activities, planning journeys, holidays, attending appointments and accompanying service users on such activities

To identify risks which may arise and involve professional staff as necessary in resolving or clarifying potentially risky situations involving service users and to report back to the Team Leader/Manager 

Requirements-

NVQ level 3

Valid driving licence

DBS (On the update service)
