Experienced Qualified Social Worker

Seven Social Care are seeking an experienced and highly qualified Social Worker to join our dedicated team in Telford. The ideal candidate will have a strong background in social work, with a proven track record of delivering high-quality services to vulnerable people.

Responsibilities:

1. Conduct thorough assessments of the social and emotional needs of service users.

2. Develop and implement personalised care plans, ensuring they adhere to statutory guidelines.

3. Conduct regular reviews to measure the effectiveness of care plans and make adjustments as necessary.

4. Collaborate closely with other health and social care professionals to ensure a multi-disciplinary approach to care.

5. Identify and report any risks present, ensuring safeguarding procedures are followed.

6. Provide crisis intervention and engage in problem-solving to alleviate immediate issues of concern.

7. Offer support, guidance, and advice to service users and their families.

8. Maintain accurate, confidential records of all interactions and interventions.

9. Participate in regular team meetings and contribute to staff development initiatives.

10. Comply with all legal and ethical guidelines related to social work.

Qualifications:

1. A Bachelor’s degree or above in Social Work.

2. Registration as a Social Worker with the Health and Care Professions Council (HCPC) is mandatory.

3. A minimum of three years of post-qualification experience in a social care setting.

4. Knowledge of relevant UK legislation and guidelines related to social work.

5. Excellent written and verbal communication skills.

6. Strong problem-solving ability and resilience in dealing with challenging situations.

7. Proficiency in maintaining accurate case notes and preparing reports.

8. Ability to work both independently and as part of a multi-disciplinary team.

9. Commitment to continuous personal and professional development.
