Family Support Worker

Seven Social Care is looking for a Family Support Worker at London Borough of Merton Council

The role:

We are looking for a motivated and experienced family support worker to facilitate high quality and comprehensive whole-family needs assessment and to lead, develop and implement a tailored support plan together with the family to meet the identified needs.

The candidate will need to be highly skilled in building meaningful and effective relationships with families. 

Key Responsibilities:

      1. Lead on carrying out whole family assessments and undertake the development of action plans with a high level of competency

 

2. Take a lead in working with families, whose needs are of a high complexity that include a wide range of factors that require a specific and individualised response through delivery of group based or 1 to 1 interventions/programmes

 

3. Be the lead contact with other identified agencies, such as Education, Children’s Centres, Community and Voluntary sector, children and adults Social Care, Housing, Health and Criminal Justice agencies. Coordinating the gathering and sharing of information and chairing multi agency meetings

 

4. Developing excellent working relationships leading to improved intervention and long term outcomes for families and their children/young people

 

5. Lead and deliver evidenced based interventions through a case work model in the family home, coordinating  and managing planned targeted interventions in a structured environment, offering both support and challenge to change family behaviours, underpinned by Merton’s Early Help practice model and motivational interviewing techniques

 

The range of case work interventions may include:

 

Ø Supporting and promoting bonding and attachment

 

Ø Promoting child development

 

Ø Carrying out practical tasks that support family wellbeing

 

Ø Assisting with daily/school routines

 

Ø Assisting with setting consistent boundaries with their children

 

Ø Supporting the family in regard to concerns of violence within the home

 

Ø Supporting the family in regard to the child’s SEND

 

Ø Supporting multiagency safety plans with family members at risk

 

Ø Helping the family manage their budget and ensuring they pay bills on time

 

Ø Setting up and attending appointments- medical, dental, benefits

 

Ø Accessing health provision

 

Ø Advice and support with education and employment

 

Ø Mediating between family members or family and agencies/neighbours

 

Ø Supporting debt and repayment schemes
