Seven Social Care is looking for a Family Wellbeing Case Practitioner at London Borough of Brent Council

The role:

The Family Wellbeing case practitioner’s role is to be proactive and dynamic in building effective and meaningful relationships with children, families and other agencies, that facilitate high quality and comprehensive whole-family needs assessment and to lead, develop and implement a tailored support plan together with the family to meet the identified needs.

The role requires extensive knowledge and skills in best practice in this field of work, and requires effective and direct close working with children, young people and adults (aged 0-25) and their families / key support networks. Providing advice, guidance and practical support that is impactful and long term, and underpinned by evidenced based practice and research

The Family Wellbeing case practitioner  will take on the role of lead professional, engaging intensively with a caseload of 12-15 families who have multiple and complex needs and  leading / coordinating support services across a multi-agency network of professionals within  both children and adult services with the aim of reducing the full range of risk factors that may lead to developmental delays, statutory social care involvement, high risk behaviour, non-attendance at school for children and other vulnerabilities as described in the eligibility criteria for access to the service.
