FAST Social Worker

Seven Health & Social Care are currently seeking a compassionate and dedicated FAST Social Worker in Lincoln, UK. The candidate will work as part of an enthusiastic team, providing essential support and services to individuals facing various forms of crises.

Responsibilities:

1. Provide comprehensive assessments specifically designed to manage the immediate crisis and identify the short-term interventions necessary to stabilize the situation.

2. Maintain an active caseload, conducting home visits, and coordinating treatment plans for individuals.

3. Work collaboratively with multi-disciplinary team members, including mental health professionals, doctors, and other health service providers to offer integrated care.

4. Identify, review, and respond to service user risks, whilst promptly addressing safeguarding issues.

5. Extensive case documentation, including evaluation, treatment, and transition plans, while maintaining client confidentiality.

6. Respond to crisis situations and provide immediate social work assistance.

7. Advocate for clients and coordinate appropriate resources/services both within and outside of the agency.

8. Develop and maintain professional relationships with clients and their families.

9. Conduct group and family meetings to discuss and address any issues related to the client’s development and well-being.

Qualifications:

1. Must hold a Bachelor’s degree in Social Work (BSW), Master’s degree in Social Work (MSW) is desirable.

2. Registration as a Social Worker with the Health and Care Professionals Council (HCPC).

3. A minimum of 2 years’ experience as a crisis or FAST Social worker.

4. Excellent communication and interpersonal skills.

5. Knowledge of community services and resources.

6. Ability to handle crises and stressful situations.

7. Ability to maintain confidentiality, integrity, and ethical guidelines of the profession.

Please note: All applicants must have the right to work in the UK, and a full driver’s license is desirable. A criminal background check will also be required.
