First Response Team – Social Worker

Seven Social Care are looking for qualified and dedicated professionals for the position of a Social Worker to join our First Response Team in Bridgwater, UK. The successful candidate will provide a comprehensive service to our clients in the community, primarily focusing on immediate needs and emergency situations.

Responsibilities:

1. Respond to emergency cases and carry out initial risk assessments 2. Create and implement effective social care plans for individuals and families, providing them with the support and assistance they need.

3. Liaise with other agencies and professionals on behalf of families and individuals.

4. Monitor, review and update care plans to ensure they meet the client’s current needs.

5. Identify resources and services that can improve the functioning and adjustment of individuals and families.

6. Provide counselling and guidance to clients helping them to deal with their situation.

7. Educate community members about the resources available for their support and the support of their families.

8. Document all consultations, treatments, and client progress and maintain accurate, up-to-date records.

Qualifications:

1. Must have a degree in social work and be registered with the HCPC (Health and Care Professions Council).

2. Proven experience working in a social work role, preferably in first-response, emergency, or crisis management settings.

3. Excellent communication, interpersonal and problem-solving skills.

4. Detailed understanding of the Social Services and Well-being (Wales) Act 2014.

5. Ability to work well under pressure and respond effectively to crisis situations.

6. Understanding and commitment to promoting and safeguarding the welfare of individuals and families.

7. Willingness to work flexible hours and available for emergencies.
