Health & Wellbeing Assessor

Seven Social Care is looking for a Health & Wellbeing Assessor to fill an exclusive opportunity in Croydon.

 We have opportunities available for up to 12 months and the pay is £24.96 per hour.

 The Hiring manager is looking for someone who can work 36 hours a week (various shifts available from Monday to Sunday).

The role:

Assessing and reviewing needs of people who are discharged from hospital via the Discharge to Assess (D2A) process. Identifying reablement needs and goals. Assessing for simple equipment e.g, commodes, and handrails.

Key Requirements:

Assessment skills

Experience of working with vulnerable adults

Basic IT skills
