Homeless Intervention Social Worker

Seven Health & Social Care are currently seeking a dedicated Homeless Intervention Social Worker for a full-time role in Worcester. The selected candidate will work closely with people experiencing homelessness, aiming to increase their wellbeing, help meet their needs, and enhance their quality of life. This role would be best suited to a compassionate, patient, and empathetic individual with a passion for serving vulnerable populations.

Key Responsibilities:

1. Lead assessments of homeless individuals to identify their needs, risks, and eligibility for support services.

2. Develop, implement, and evaluate person-centred care plans based on assessments.

3. Coordinate access to services such as housing, mental health support, substance abuse treatment, medical care, and food provision.

4. Provide crisis intervention and advocacy services when required.

5. Maintain accurate and up-to-date client records, whilst adhering to privacy standards and regulations.

6. Establish and maintain partnerships with local services and providers.

7. Conduct outreach work and build trusting relationships with clients.

8. Train and guide staff and volunteers to provide effective service to the homeless population.

9. Participate in team meetings, training courses, and other professional activities as required.

Qualification and Experience:

1. A degree in Social Work, Psychology, Sociology or related fields.

2. Registered as a social worker with the Health and Care Professions Council (HCPC).

3. Proven experience in working with socially excluded and vulnerable individuals, preferably those experiencing homelessness.

4. Knowledge of the social and structural issues affecting homeless individuals.

5. A sound understanding of safeguarding issues, confidentiality, and data protection.

Skills:

1. Excellent communication, problem solving, and negotiation skills.

2. The ability to empathise with clients and maintain professional boundaries.

3. Teamwork skills & ability to work independently with minimal supervision.

4. Ability to manage stressful situations and deal with challenging behaviour.

5. Strong organisational skills and ability to manage multiple tasks at once.

6. Proficiency in record keeping and report writing.

Please note, the successful candidate must be able to work flexible hours, including some evenings and weekends. A current DBS Check will be required.
