Hospital Discharge Social Care Facilitator

Seven Resourcing is working with Birmingham City Council to recruit a Social Care Facilitator for the Hospital Discharge service in Woodcock Street.

Initial 3 month contract with the onlook of extension.

£12.97 per hour.

Monday – Friday 9am – 5pm 37 hours per week, but if you need flexibility on days/hours please specify on your CV.

Duties:

The role will be to maintain data records, working with SW’s and be working under Senior Practitioners to support cases and working on discharge. Be part of the wider admin team. The applicants will need to be flexible in covering other duties when required.

Applicant Requirements:

• Experience: 2+ years working within a similar role.
• Compliance: Current DBS check and references covering the last 3 years.
• Travel: Clean UK driving license with the ability to commute to and from work independently.
• Hours: 37 hours p/w.
