Hospital Discharge Social Worker

Seven Social Care are currently seeking an experienced and dedicated Hospital Discharge Social Worker to join our team in Dudley, UK. The key objective of this role is to help patients transition from a hospital setting into a safe and suitable environment following discharge. 

**Responsibilities:**

1. To execute comprehensive assessments of patients, including their medical, emotional, and financial circumstances, to establish a discharge plan that meets their needs.

2. Collaborate effectively with a multi-disciplinary team of doctors, nurses, and other healthcare professionals to ensure a patient’s smooth transition from  hospital care.

3. Provide emotional and practical support to patients and their families, including counselling and referrals to appropriate resources (financial assistance, home health care, transportation, etc.) 4. To advocate on behalf of patients and families to ensure their needs are met and their rights are respected.

5. Keeping track of each patient’s progress, making adjustments to the discharge plan as needed, and solving any issues that arise.

6. Stay informed on current legislation, national standards, and best practices to ensure optimal patient care.

7. Managing paperwork and keeping thorough records of patients’ cases.

**Qualifications:**

1. A degree in social work, nursing, or any related field is required.

2. Registered with the Health & Care Professions Council (HCPC).

3. Proven experience in a hospital or healthcare setting.

4. Proficient knowledge of medical terminology and hospital procedures.

5. Understanding of national standards and legislation related to patient care after discharge.

6. Excellent communication and negotiation skills.

7. A caring and compassionate character with a passion for assisting others.
