Hospital Discharge Social Worker

Seven Social Care is working with Dudley Council to recruit a Hospital Discharge Social Worker within a Mental Health Service at their Council House Office.

£32 per hour via umbrella company

Job

1.    Ensure the service user and carer have opportunity to have the shortest length of stay in institutional environment as possible. 

2.    Discharge will be efficient and effective, with appropriate personalised support ensuring user can be maintained at home and carer is considered and supported within any package of care, ensuring individual flexibility for support where possible and personalised planning aided by VSC support worker is completed. 

3.    Key contacts include NHS organisations; Black Country Health Care Trust, Acute Hospitals as well as any community mental health or physical health teams. Local authority SW Teams and VSC providers.

4.    Duties include:

a.    Able to complete care act assessments and make recommendations – start the process of such recommendations 

b.    Expectation to complete capacity assessments if required.

c.    Regular attendance to ward reviews / pre discharge meetings / 117 reviews where appropriate

d.    Regular link up with housing providers and external support agencies to facilitate discharges 

e.    To commit to regular meetings with discharge coordinator (health) 

f.    Be a point of contact to facilitate a smooth process to access social care support, and able to escalate accordingly and provide BCH with appropriate timescales and updates 

g.    Ensure effective communication with locality adult social care.

h.    Explore options of respite / step down 

i.    Attending funding panel, make recommendations, present cases

j.    Support any s117 funding tool applications as required.

To act as a bridge between organisations and individuals involved in the support of the service user, to assist in developing effective working relationships between health and social care
