Hospital Discharge Support Worker

Seven Social Care are looking for a Social Work Practitioner to work within the Hospital Discharge Team in Hebburn. This contract is initially for 3 months and likely to be extended for longer.

The role:

The role does not require a Social Work qualification however social care experience is needed ideally around the Discharge to Assess model currently being utilised to ensure safe discharge of people identified as having care and support needs as they leave hospital. Knowledge of the Care Act 2014 and Mental Capacity Act 2005 is required. Administration skills in terms of Microsoft Office, Outlook and Liquid Logic Adults would be beneficial including confident ability to converse over the telephone with people and other professionals. Ability to work as part of a team and also autonomously is essential. Some visits to see people at home may also be required.

The post is located at Clarendon House, Windmill Way, Hebburn NE31 1AT. Agile working can be considered but not all the time to ability to commute to Clarendon daily is required.

Key Responsibilities:

      To use active listening skills, observation and communication to build relationships with adults, their families and informal network support and provide advice and guidance to help support them to navigate their way through systems.

To liaise with people, their families, carers and key health professionals to ensure a seamless discharge process.

Adopt a home first or discharge to assess way of working to ensure people are discharged from hospital in a safe and timely manner with a focus upon rehabilitation and recovery, connecting people to their communities.

Applicant Requirements:

  • Qualifications: Level 3 in Health and Social Care
  • Experience: 2+ years working in a similar role
  • Compliance: Up to date DBS.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37.5 hours p/w.
