Independent Living Facilitator

Seven Social Care is looking for a Independent Living Facilitator at Cambridge City Council

Job Purpose

To provide support to older people in Cambridge and to ensure sheltered housing schemes are safe and well maintained.

Service Roles & Responsibilities: 

To assess the needs of and provide support to older people in Cambridge, completing assessments, reviewing and redrafting support plans as required.

To undertake premises inspections of the sheltered housing schemes, to order and liaise with contractors regarding communal repairs, cleaning and other services and to empower and report residents repairs as needed.

To ensure that partner agencies and contractors records are updated and that any special needs are communicated and recorded.

To ensure clients are visited in accordance with an agreed support plan and to refer and report their needs to agencies as required.

To liaise with and refer to other professionals as required such as social workers, OT’s, community nurses, GP etc
