Inreach Navigator

Seven Social Care are currently recruiting for several Inreach Navigators to start a new role in Manchester. 

Inreach Navigators carry a case load of 10 – 12 clients who are based in temporary accommodation across the city. These clients will have support needs that Navigators will support clients with.

Support needs will vary between each case but will include, mental health, physical health, adverse childhood trauma, drug, and alcohol addiction.

Navigators will be responsible for working with other agencies, chairing MDT meetings, and coordinating the support needs for the individual.

Responsibilities:

Establish and maintain relationships with people to challenge their existing patterns of behaviour.  Work proactively with appropriate stakeholders to assess the individual’s needs and ensure positive outcomes.

Provide support and advice to people which provide the opportunity to engage with appropriate local and national organised and community-based services.

Monitors evaluate and review assessments as required determining suitable adjustments to ensure that people are able to achieve their agreed goals.

Work with colleagues and stakeholders to produce plans that use the person’s individual needs to identify and reduce their vulnerability to risk situations.

Applicant Requirements:

Experience:  Previous experience working with mental health, physical health, adverse childhood trauma, drug and alcohol addiction.

Compliance:  Enhanced DBS, Right to Work in the UK

Hours:  35 hours a week, Monday to Friday – office based

Travel: Full UK driving license and access to car (desirable)
