Key Worker

Seven Social Care is looking for a Key Worker at London Borough of Haringey Council

The role:

Working with the young people, their family where appropriate, prospective carers, relevant professionals and staff, plan and implement individual plans. Taking in to account the needs, choices and wishes of the young person in the creation of the plan to promote identified needs being met. Review and evaluate plans regularly and amend as appropriate.

Ensure that young people and their families are appropriately informed and involved about aspects of their life within the home and local community,

Support young people with all aspects of developing appropriate life skills; communication, daily personal self-care including hygiene, domestic chores, attendance at various appointments and support on meaningful activities outside of the home. T hese discussions should contribute to the residents’ sense of purpose, belonging and achievement.

Follow all policies & procedures associated with young people & Haslemere including (but not limited to) safeguarding, Health & Safety, medication management, pocket money management and ensure the appropriate records are completed accurately and kept appropriately.
