Occupational Therapist

Seven Social Care is looking for an Occupational Therapist to fill an exclusive role for Solihull Metropolitan Borough Council. The pay rate for this is £28 per hour.

Job Role:

To promote, enable and support independence and maintain health and wellbeing through reablement, enabling independence, prevention, retaining a person centre approach and a strengths-based practice. 

Key Responsibilities:

• Supporting community based residents from a diverse background of needs but likely to involve those recently discharged from hospital and in need of social care support.
• Proportionate assessment within 48 hrs of discharge to home/care home/extra care to include as appropriate: outcome focussed goal setting, therapy plan devising, outcomes/progress reviewing, home environment assessment and provision/prescribing of minor equipment, provision of minor home adaptations or onward referral for major adaptations.

• To work alongside people with wide range of disabilities including, physical, learning and mental health disabilities, providing a planned programme of therapeutic intervention to maximise functional ability, to enable the person to achieve optimum control of their lives, reduce dependency and increase independence. The post holder will be responsible for undertaking specialist functional assessments in order to establish if people meet criteria for the provision of services.
• Where necessary, to undertake environmental ergonomic assessment of the person’s environment; assessment of cognitive and functional ability to carry out daily living tasks; assessment of manual handling situations. To work alongside people, carers and staff providing a planned programme of interventions which maximises functional ability with the desired outcome of reducing dependency and promoting independence.
• To be responsible for providing a therapist specialist advisory and guidance role to staff, professionals, formal and informal carers. Where necessary, to participate in project work and to be responsible for a range of liaison functions.
• To make individual home visits to assess people’s needs in accordance with current legislation and anti discriminatory practice, promoting self directed support and ensuring demonstrable outcomes.
• To undertake specialist manual handling risk assessments, advising staff, clients and carers on techniques, equipment and methods of reducing risk.
• To discuss with people the outcome focussed care plan from their assessment, identifying the choices and options to meet their needs and to maximise their independence, ensuring at all times that value for money is achieved.
• To plan an agreed programme of therapeutic intervention with agreed goals and to ensure that the plan is carried through.
• To work in conjunction with people, their carers, other professionals and agencies, to coordinate and implement agreed service provision.
• To provide equipment as appropriate to assist with daily living and to be proficient in the instruction and demonstration of equipment to aid daily living to service user and carers to enable them to achieve maximum functional independence
• To maintain an up to date knowledge of medical conditions and treatment, disability issues, legislation and departmental policies and practices and to liaise with colleagues and manager regarding modifications in practice that may be required as a result.
• Keep up to date with legislative changes and best practice in the field of Occupational Therapy.  
• Where appropriate, to provide day to day clinical support to OT Assistants/carers and to supervise and guide therapy assistants in undertaking assessments and intervention plans.
• Maintain detailed and accurate case records and provide written reports related to people’s services and/or carers needs when appropriate.
• To raise awareness of the needs of people with a disability including sensory impairment by networking with other professional colleagues and other groups as required.
• To participate in regular supervision and consultation with the Line Manager and to ensure that the Line Manager is made aware of all issues of major importance or concern (in the absence of the Line Manager this should be another relevant manager).
• To manage assigned work in accordance with Directorate policies and procedures contributing to the overall performance of Directorate including leading on predetermined projects and taking cases of a more complex nature, offering support and supervision to less experienced staff as appropriate.
• To work, at all times, within the professional ethics and disciplines of appropriate professional bodies.
• Any other duties that may be required as commensurate with the grading of the post.
