Occupational Therapist (Adults)

Seven Social Care is looking for a qualified Occupational Therapist to fill an exclusive opportunity in Doncaster.

The role:

Promote independence and reduce risks for Adults with disabilities living in Doncaster. Work with the social model of disability and use evidence based practice to carry out person centre assessments to identify need and take appropriate action. Use person centred methods that respect individual values, cultures and diversity to identify needs and devise individual interventions to meet need. Where statutory provision is not appropriate offer information and advice around self-directed support.

There is a requirement to provide a high quality, evidence-based, Integrated Community Equipment and Adaptations Service to people in their own homes which will be achieved by carrying a complex caseload and using a robust performance management framework to ensure effective service delivery by all officers working in the service.

This service is target driven with a clear focus on activity and performance. The post holder is required to meet performance targets, actively participate in the data capture process and report to the Deputy Principal OT / Principal OT when targets are not met.

Key Responsibilities:

Carry out needs led assessment of occupational performance with Clients who have a wide range of medical conditions and physical disabilities. Acknowledge and analyse physiological, psychological and environmental factors to develop individual goal orientated action plans.

Take into account the views of the person and their carer(s) to agree appropriate provision of equipment, minor and major adaptations or re-housing that is necessary to meet current and long-term needs whilst considering the legal framework, feasibility, practicality and resource implications of the proposed plan. Have particular responsibility for people with the most complex needs.

This post will have particular clinical responsible as locality lead to work across organisational boundaries to deliver integrated housing solutions to meet the long term needs of the most complex Adults. This will involve innovative solutions outside of traditional solutions to ensure best value in the longer term.

Carry out holistic assessments and take action to reduce and manage risk to people in their homes advising them and their carer(s) on various aspects of safe independence or care. Work in collaboration with assessment and care management to ensure care provision is appropriate post intervention i.e. care package is reduced if independence is increased.

Applicant Requirements:

  • Qualifications: Degree approved by the Health and Care Professions Council (HCPC).
  • Experience: 2+ years working with children as a qualified Social Worker in the UK.
  • Compliance: Up to date HCPC registration and DBS.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37.5 hours p/w.
