Outeach Engagement Officer

Seven Social Care are currently recruiting for two Outreach Engagement Officer’s to start a new role in Manchester.

As an Outreach Engagement Officer, you will undertake home visits and carry out holistic strengths-based observations and assessments of citizens to support a determination of social care needs.

You will arrange timely support for reablement, packages of care, respite, carer assessments, equipment, and minor adaptations to support independent living, assistive technology and support carers and family members.

This is a full time contract initially for 7 months with the view of extending, paying £21.08 an hour.

Responsibilities:

• Advising citizens and referring them to appropriate services based on the observation and assessment completed, ensuring that the provision of services supports citizens maximise their independence.
• Developing and implementing citizen support plans to meet identified social needs of individuals, enabling the citizen to achieve maximum health and well-being.
• Establishing and maintaining relationships with citizens and working proactively with appropriate stakeholders to assess the individual’s needs and support positive outcomes for citizens.

Applicant Requirements:

• Experience:  Applicants must have experience of working with people who use substances to dependent levels. Consideration will be given to applicants with outreach/support worker experience with people with complex mental health
• Qualifications: NVQ Level 4 in Health and Social Care
• Compliance: Enhanced Adults and Childrens DBS, Right to Work in the UK
• Hours: 35 hours a week, office based with visits
• Travel: Full UK driving license and access to a car
