QSW – Adult Community Team

Seven Social Care are currently looking for an experienced Qualified Social Worker to join our dedicated Adult Community Team in Eastleigh. The successful candidate will have a passion for making a significant impact on the lives of adults, particularly those who require social care services.

Responsibilities:

1. Deliver high-quality care services as part of the Adult Community Team, which includes handling complex caseloads.

2. Collaborate with a multi-disciplinary team to assess, plan and review the needs of adults and their families.

3. Implement, evaluate and revise care plans based on each individual’s requirements and circumstances.

4. Regularly liaise with other agencies and community resources to ensure a comprehensive support network is in place for each client.

5. Maintain up-to-date records and report issues, changes, or progress accordingly.

6. Participate in training, meetings and conferences as required to enhance professional development and stay updated with current practices and legislation.

7. Comply with all current UK legislation and standards pertaining to social work.

Qualification and Requirements:

1. A recognised Social Work degree, or equivalent.

2. Registration with Social Work England.

3. Experience of working within an Adult Social Care setting.

4. Possess excellent communication and multi-tasking skills.

5. The ability to handle sensitive issues with discretion and respect for confidentiality.

6. Excellent problem-solving skills and resourcefulness.

7. A strong commitment to delivering an excellent standard of care and ability to work cooperatively within a multidisciplinary team.

An enhanced DBS check will be required for this role.
