Qualified Social Worker-Adults- Community Team

  • Salary: £32 - £35 per hour

Seven Social Care are currently seeking for a dedicated, empathetic, and qualified Social Worker to join our Adults – Community Team in Yeovil. This position involves working closely with adults within the community, offering them crucial support in times of need.

Key Responsibilities:

1. Work effectively as part of the community-based team to assess, review and provide support to adults, their families, and carers.

2. In partnership with service users, develop person-centred care plans that promote self-care and independence.

3. Act as the key worker for a caseload of service users, providing appropriate interventions and reviewing progress.

4. Establish and maintain effective relationships with various stakeholders, including service users, family members, and other health care professionals.

5. React and manage safeguarding issues within the community, undertaking inquiries and implementing protection plans as required.

6. Identify various community resources and support networks available for service users and guide them to access these services.

7. Provide supervision and coaching to support workers and student social workers within the team.

Qualifications and Skills:

1. A degree in social work along with a current registration with the Health and Care Professions Council (HCPC) or Social Work England.

2. Significant experience in adult social work, especially involving work with vulnerable groups.

3. Demonstrable skills in assessment, care planning and reviewing care plans.

4. Knowledge and understanding of current legislation related to adult social care, such as the Care Act, Mental Capacity Act and safeguarding processes.

5. Excellent communication skills, both verbal and written; and the ability to handle sensitive information with discretion.

6. Ability to manage complex caseloads, meet deadlines, and work under pressure.

7. An empathetic approach with the ability to build and maintain effective relationships with service users, their families, and carers.

8. A current, valid driver’s license and access to a vehicle for work purposes.
