Reablement Worker

Seven Social Care is looking for a Reablement Worker at London Borough of Hammersmith and Fulham Council

The role:

• To provide a high standard of reablement care and non invasive health treatment for clients referred into the Community Independence Service.
• To work in a multi agency environment with clients to instil a reablement service focus.  To provide care which focuses on the needs of the individual, promotes independence and choice and maximises quality of life. 
• To avoid admissions to more institutional forms of care.  
• To provide a dignified and respectful service that encourages and motivates disabled, frail and older people to live active lives of their choosing.

General

To participate in the monitoring of quality standards such as providing client feedback questionnaire forms and support clients to complete them as relevant.

To assist in the induction of new team members with particular reference to systems training, advice and guidance.

To comply with all Human Resources data recording requirements including sickness returns on resource link and REAL performance reviews.

To offer peer support, skills sharing and training within the team as required.

To participate in training and make constructive use of supervision and performance review to enhance opportunities for personal development, and to contribute improvements to the service
