Rehabilitation Assistant

Seven Resourcing are looking for a Rehabilitation Assistant to work in Great Western Court within the rehabilitation team for Gloucestershire County Council. We are paying £16.13 per hour for this role, and is 21 hours a week. 

The role:

  • To deliver a service that provides a range of rehabilitation and care programs that promote independence.
  • To assist the Management Team in the day to day running of the service in line with Care Quality Commission Essential standards of quality and safety.
  • To promote the independence and reablement of service users, each of which will have an outcome focused care plan, developed by the multidisciplinary team to meet identified needs
  • To work across Health and Social Care boundaries for a time-limited period, preventing avoidable hospital and facilitating earlier discharge.

Key Responsibilities:

 

  • To contribute to the formulation, implementation and review of care/support plans in conjunction with the Multidisciplinary Team (MDT).
  • To develop and maintain effective relationships with all people involved in each individual’s care/support plan.
  • To consider, with senior staff, risk assessment issues and implement detailed risk management guidelines.
  • To assist in the management of emergency situations.
  • To maintain health and safety in the workplace.
  • To report incidents/situations and allegations of abuse in accordance with departmental and local policy guidelines.
  • To implement and work to all local policies and procedures.
  • To participate in Supervision and Performance Assessment Review process, including identifying own training and development needs.
  • To contribute to the MDT value base, standards and strategic direction, including putting forward ideas for improvement.

Applicant Requirements:

  • Qualifications: GCSEs A-C or equivalent including at least English and Maths.

    Educated to A Level standard or equivalent experience

  • Experience: 2+ years working within a similar role. 
  • Compliance: Current Standard DBS check. 
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37 hours p/w.
