Senior Social Worker – Contact and Assessment Service

  • Location: Hammersmith & Fulham, Greater London
  • Salary: £27 - £27 per hour
  • Type: Temporary / Full Time

Responsibilities:

1. Carry out assessments of the social and care needs of individuals, families and carers, and manage cases where necessary.

2. Maintain professional relationships with individuals, families and carers to assess their needs and resources, while maintaining confidentiality and privacy.

3. Develop and implement robust care plans and strategies, ensuring that appropriate resources are utilised to maintain the welfare of service users.

4. Provide expert advice and assistance to contact and assessment service users at all times.

5. Participate in regular team meetings and contribute feedback on the improvement of the service.

6. Monitor and evaluate service user’s progress, supervise their execution of agreed tasks and coordinate reviews as necessary.

7. Work collaboratively with colleagues and relevant agencies, such as health services and schools, to meet the needs of the service users.

8. Comply with standards as set out in legislation and regulatory bodies, maintaining an up-to-date knowledge of best practices and policies.

9. Provide mentorship, guidance, and supervision to newer or less experienced staff members.

10. Assist in training activities, seminars, and workshops within the department and the larger community.

Requirements:

1. Degree in Social Work or equivalent level of qualification.

2. Registration with the Health and Care Professions Council (HCPC) 3. Minimum of 5 years experience working in a social work environment, with at least two years in a senior role.

4. Excellent verbal and written communication skills.

5. Experience in dealing with a diverse range of people, issues and capacities.

6. Demonstrable knowledge of social work theory and legislation, including safeguarding.

7. Exceptional team working skills, with the ability to work autonomously when required.

8. Able to manage workload effectively and meet deadlines.

9. Strong decision-making skills with the ability to undertake assessments and manage risk.

10. Must have a clear DBS and ability to work in the UK.
