Social Care Assessor

Seven Social Care is looking for a Social Care Assessor to start a new exclusive role in Manchester. This is a temporary contract initially for 6 months with the possibility of placement extension.

The role:

You will undertake home visits and carry out holistic assessments and observations of customers, in order to determine customer needs. You will also advise customers and refer them to appropriate services based on the assessment and observation carried out, ensuring the provision of appropriate services to ensure customers are supported in a way that maximises their independence.

Key Responsibilities:

Carry out holistic assessments and observations, identifying and arranging appropriate adults’ services to meet the needs of the customer.

Contribute to the single assessment process by working collaboratively with appropriate partners, in order to provide an efficient service, achieving outcomes as required by customers.

Monitor, review and evaluate assessments as required, determining suitable adjustments to ensure customer needs are met at the highest standard.

Provide and facilitate the provision and fitting of equipment, adaptations and/or intervention from other agencies or departments.

Applicant Requirements:

Experience: Experience of working in a Social Care or Health Care Setting with an understanding of relevant Social Care legislation.

Qualifications: Hold or willing to undertake and achieve NVQ Level 4 in Health and Social Care

Compliance: Up to date enhanced DBS

Hours: 35 hours a week
