Social Care Assessor

Seven Social Care is currently seeking an experienced Interim Social Care Assessor to join our client’s team in Ellesmere Port.

The successful candidate will assess/review service users who are discharged from hospital with care to identify alternative or continued support requirements in line with relevant legislation i.e. Care Act.

Key Responsibilities:

Conduct assessments of individuals’ social care needs in line with relevant legislation, policies and procedures.

Work collaboratively with service users, their families, carers and other professionals to develop and implement care plans.

Monitor and review care plans regularly, ensuring that they remain appropriate and effective.

Provide support and advice to service users, their families and carers on social care issues.

Work with colleagues and partner agencies to coordinate the delivery of appropriate support and services.

Maintain accurate and up-to-date records of assessments, care plans and other relevant documentation.

Participate in the development and implementation of policies and procedures to ensure high-quality service delivery.

Applicant Requirements:

Experience: Previous experience of working in a health or social care environment or similar setting including working with people with adult care needs in their own home.

Compliance: Up to date enhanced DBS on the update service, Right to work in the UK.

Hours: 37 hours a week Monday to Friday 

Travel: Ability to travel to different locations (car user preferred)
