The role:

The post holder is directly accountable to the Team Manager.  Duties will involve contact with staff at all levels throughout the Department and other Council Departments, and with staff of other agencies, including statutory and voluntary.

You will carry out your work to the standards set in the relevant professional and national occupational standards.

    To provide a service to older people, people with disabilities and their carers in the community and/or the county hospitals, within the framework of relevant legislation, Council Policies and procedures. This follows an early intervention and prevention model so advice, information, and the least restrictive options are considered first.
    The service and intervention provided is person centred and aims to support people to maintain their independence and where possible remain living in their own homes and communities. Good customer service is required with all actions agreed followed through.
