Social Care Practitioner – Mental Health Team

Seven Resourcing are looking for a temporary Social Care Practitioner to work in the Mental Health Team in Jarrow. This position is initially for a period of 3 months and is paying £11.70 – £14.62 per hour (PAYE and umbrella payment options are available). 

The role:

The role is to work within the mental health team in providing support to a diverse group of people by way of working to support people to stay safe and to carry out work in a timely manner, through a successful partnership between Cumbria, Northumberland, Tyne and Wear NHS trust, Social Care, and our partners in the third sector, the police, probation, and ambulance services.

You will be working collaboratively with the team in daily huddles to identify opportunities in supporting people with complex mental health needs, in areas such as housing, drug and alcohol services, volunteering and other third sector providers. Good communication, time keeping and prioritisation skills are essential in this role.

There is a requirement of this role to carry out duties in the most flexible, effective and efficient manner to support the service users their carers and  colleagues.

Key Responsibilities:

  • To use active listening skills, observation and communication to build relationships with adults, their families and informal network support and provide advice and guidance to help support them to navigate their way through systems.
  • To liaise with people, their families, carers and key health professionals to ensure a safe service
  • Ensure there is access to care and support if needed on the day of discharge from a hospital setting, and work directly with the person at home or in a community setting to support them to undertake an assessment of needs using our Let’s Talk Together approach and enabling them to identify the outcomes they wish to achieve in.
  • To participate in allocations meeting and to take cases that requires safeguarding enquiries  within section 42 of the Care Act 2014
  • To work closely with health, housing and voluntary sector/the police and colleagues to ensure resources are used effectively across the system. Multi-disciplinary working will be critical to the success of this role.
  • Jointly agree a suitable support plan/safeguarding plan which may include equipment and technology as well as community resources to get back to their daily routine.
  • To undertake a timely review post assessment, ensuring the right level of support is only provided for as long as is needed to support recovery.
  • To work in accordance with safeguarding policies, safeguarding individuals in a way that supports them in making choices and having control. Ensuring that the ‘Making Safeguarding Personal’ ethos is at the heart of all practice.
  • To develop an awareness of services available in the community, and understand the roles and accountabilities of other agencies and professionals to refer, liaise, consult and work effectively in order to meet identified needs.

  • Qualifications: N/A. 
  • Experience: 2+ years working within a similar role.
  • Compliance: Current DBS check and references covering the last 2 years. 
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37.5 hours p/w.
