Social Worker (Adults)

Seven Social Care are currently seeking a compassionate, ethical, and dedicated Social Worker for adults to join our team. This position will involve working directly with adults to assess their needs, social circumstances and support them through personalised care and providing the necessary resources to ensure their well-being.

Job Responsibilities:

1. Assess and evaluate the needs of adults in care to create personalized care plans.

2. Coordinating with medical professionals, mental health professionals, and other social workers to ensure all needs are met.

3. Provide advice and guidance to individuals and families about available services and resources.

4. Monitor and review care plans regularly, adjusting them as needed based on changes in circumstances.

5. Advocate for client rights to ensure their protection and safety at all times.

6. Conduct risk assessments to identify any potential risks related to their health and wellbeing.

7. Collaborate with other agencies involved with your clients’ welfare.

8. Maintain accurate records and prepare reports for legal action.

9. Attend multidisciplinary meetings about clients involving health and social services.

10. Comply with all relevant legislation, policies, and procedures while maintaining the highest professional standards.

Job Requirements:

1. A qualified Social Worker (SW England registration required).

2. Able to work with vulnerable adults and uphold their rights.

3. Have a clear understanding of the UK legislative environment for adult social care.

4. Excellent communication, organisational and report writing skills.

5. Proven ability to work effectively both independently and as part of a team.

6. Able to maintain high levels of confidentiality and to exercise discretion.

7. Experience in dealing with multi-disciplinary teams in health and social care.

8. Comfortable making difficult decisions, with a demonstrable ability to cope under pressure.

9. Use of your own vehicle for work purposes, and a clean driving record.
