Social Worker – Adults Acute Hospital Team

  • Salary: £35 - £35 per hour
  • Type: Temporary / Full Time

Seven Social Care are currently seeking an experienced and dedicated Social Worker to join our Adult Acute Hospital team in Chippenham. This role calls for a qualified professional who can work effectively in a fast-paced, dynamic health care environment, with a passion for helping adults during critical periods of their life.

Duties and Responsibilities:

1. Undertake assessments of individuals and families to identify needs, risks and desired outcomes.

2. Implement and review individual care plans and coordinate with multidisciplinary teams.

3. Conduct regular follow-ups to assess the progress of the patients, reassess their needs, and amend care plans when necessary.

4. Advocate on behalf of individuals and families to help them navigate through the healthcare system and access required services.

5. Work directly with adults who have complex needs, provide guidance, and empower them through informed decisions.

6. Liaise with other professionals such as occupational therapists, doctors, nursing staff and social services.

7. Contribute to service development and keep up to date with social legislation and regulation.

8. Handle and manage crisis situations whenever they arise.

Qualifications and Skills:

1. Degree in Social Work and registered with Social Work England.

2. Previous experience in a similar hospital environment or working with older adults, mental health patients, or individuals with disabilities.

3. Excellent interpersonal, advocacy and casework skills.

4. Good knowledge of social care legislation and policies.

5. Ability to handle sensitive situations with tact and diplomacy.

6. Able to handle a demanding workload effectively.

7. Highly organized, with strong problem-solving skills.

8. Eligibility to work in the UK.
