Social Worker – Adults MASH Team

Seven Social Care are looking for a dedicated and experienced Social Worker to join our Adult MASH Team based in Hillingdon. The successful candidate will work collaboratively with both internal team members and external stakeholders to ensure the wellbeing of vulnerable adults.

Responsibilities:

1. Conduct comprehensive assessments of adults to determine their needs and formulate care plans that address those needs.

2. Respond swiftly to safeguarding alerts, carrying out investigations designed to protect vulnerable adults from harm, abuse, neglect, and exploitation.

3. Collaborate and communicate effectively with multidisciplinary teams, family members, carers, and professionals from other agencies, including healthcare, police, and local authorities.

4. Participate in MASH meetings, ensuring that accurate and comprehensive reports are prepared and presented in a timely manner.

5. Advocate for service users, ensuring they are involved in decisions that affect them and their rights, dignity, and independence are respected and preserved.

6. Continuously monitor and review care plans, adjusting them as necessary to achieve best outcomes.

7. Maintain up-to-date knowledge of legislation, policies, and procedures in the area of safeguarding adults and put those into practice.

Basic Requirements:

1. A recognised Social Work qualification (e.g., Bachelor’s degree, Master’s degree, or Post-qualifying certificate in Social Work) 2. Registration with the Health and Care Professions Council (HCPC) 3. Previous Experience within an Adult Safeguarding environment.

4. Excellent interpersonal and communication skills.

5. Knowledge of the Mental Capacity Act 2005, Care Act 2014, and other relevant legislation.

Desirable:

1. Proven ability to manage complex caseloads.

2. An understanding of local and national policy and procedural frameworks relating to the welfare of adults.

3. Experience of working within a multi-agency partnership.

4. Demonstrated ability to work under pressure whilst maintaining high-quality standards of work.

The successful candidate will need a solid understanding of safeguarding adults, possess a strong commitment to the values that underpin social work, and demonstrate exceptional communication, organisational, and problem-solving skills.
