Social Worker – Adults Project Team

Seven Social Care are currently seeking a passionate and highly dedicated Social Worker to join our Adults Project Team in Trowbridge. The successful candidate will be responsible for providing an efficient and effective service to adults facing various social, physical, and mental challenges.

Key Responsibilities:

1. Conduct interviews with individuals and families to assess and review their situation.

2. Undertake and write up assessments that meet specified standards and timescales.

3. Provide support to adults suffering from different issues including mental health, substance abuse, domestic violence etc.

4. Develop appropriate care and support plans to meet the specific needs of adults in the community.

5. Collaborate with other professionals such as psychologists, GPs, psychiatrists to ensure provision of aligned and comprehensive service.

6. Regularly review, scrutinise and adjust care plans as per the changes in the circumstances.

7. Ensure all case management complies with the professional and corporate standards of record keeping.

8. Provide crisis intervention as necessary.

9. Advocate for clients or patients to resolve crises.

Essential Skills and Experience:

1. Degree in Social Work and registration with the Health and Care Professions Council (HCPC).

2. Previous experience in a similar role, preferably within adult services.

3. Excellent communication, interpersonal and organisational skills.

4. An understanding of the relevant legal and social policies and a commitment to the values and ethical principles of social work.

5. The ability to handle sensitive situations with tact and diplomacy.

6. Ability to work independently, prioritize work, manage time effectively, and meet deadlines.

7. A valid UK driving licence and access to a vehicle for work purposes.
