Social Worker – Assessment & Intervention

Seven Social Care is looking for a Social Worker – Assessment & Intervention in London Borough of Tower Hamlets

The role:

Working as part of a team, providing a range of specialist assessment and Interventions services (both centre based and outreach) specifically targeted to families with complex needs, within a diverse local community.

Undertake assessments and Interventions that are critically reflective, analytical and evidence based, utilising relevant social work tools, knowledge and theory.

In partnership with professionals in house and colleagues across agency boundaries, ensure that work is undertaken in line with agency care plans to safeguard and promote the well-being of children and young people.

Key Responsibilities:

Responding to referrals, work with children and families, both centre based and within the family home, undertaking a range of comprehensive assessments and family Interventions which focus on the needs and wellbeing of the children and young people.

Respond to allocated case work, gathering and screening information about the needs of children and families in order to ascertain and assess the level of risk and type of support required.

To act as a key worker carrying a caseload of families who are in crisis or at risk of breakdown. To assess, support, advise and Intervene in order to promote the interests and welfare of children.
