Social Worker – Assessment Team

Seven Soical Care are currently seeking a reliable and compassionate Social Worker for our dynamic assessment team. The candidate must possess significant experience in assessment, prevention, family engagement, and therapeutic interventions. Based in Hampshire, this position offers a competitive wage of £42 per hour.

Duties and Responsibilities:

1. Conduct comprehensive assessments of the needs of individuals and families, considering social, behavioural, emotional, and environmental factors.

2. Prepare and document thorough, evidence-based assessment reports.

3. Coordinate with multi-professional teams to develop and implement effective care plans based on assessments.

4. Provide direct therapeutic interventions and support individuals/families in crisis by deploying crisis intervention techniques.

5. Hold an active caseload and manage referrals effectively.

6. Advocate for client or community needs at inter-agency meetings or other forums.

7. Monitor service provision for compliance with regulations, and to ensure that interventions are meeting the goals and objectives of the care plan.

8. Develop and maintain professional relationships with clients, ensuring that their voices and wishes are central to their care.

9. Conduct regular review and update assessment and care plans based on clients’ evolving needs.

Qualifications:

1. Bachelor’s or Master’s degree in Social Work (BSW/MSW) and registered with Social Work England.

2. A minimum of two years’ experience working in a similar role, ideally within an assessment team.

3. Expertise in conducting needs assessments and creating care plans.

4. Excellent communication and interpersonal skills.

5. Ability to work with individuals from diverse backgrounds and cultures.

6. Familiarity with safeguarding issues and legal aspects of social care.

7. Knowledge of social services policies and procedures.

8. Proficiency in case management software and Microsoft Office Suite.

9. Efficient multitasking and organisational skills.

10. Ability to work flexibly, including evenings and weekends if required.

11. Full UK driving license and access to a vehicle, as the role will require some travel.
