Social Worker – Assessment Team

Seven Social Care are currently seeking a dedicated, compassionate, and skilled Social Worker to join our Assessment Team based in Tiverton, UK. The successful individual will be integral in identifying the needs of service users and making informed decisions regarding the necessary steps to improve user situations.

Responsibilities:

1. Conduct comprehensive assessments on individuals and families to evaluate their social, emotional, physical, and mental states.

2. Develop and implement effective care plans, provide referrals, and arrange community resources.

3. Liaise and collaborate with other professionals in the health and social care sector, such as doctors, psychologists, and school staff.

4. Maintain in-depth records and write detailed reports, which could be used in legal proceedings.

5. Participate in multidisciplinary team meetings to discuss cases and offer and receive support.

6. Provide emotional support and counseling to service users and their families.

7. Adhere to all company policies, procedures, and guidelines, including confidentiality and safeguarding procedures.

8. Participate in continuous professional training and development opportunities to stay updated on laws, best practices, and industry developments.

Qualifications and Skills:

1. A degree in social work and a registered member of the Health and Care Professions Council (HCPC).

2. Proven experience working within an Assessment Team in a social work setting is strongly preferred.

3. Strong knowledge of relevant law, child protection procedures, and local and national policies affecting children and families.

4. Excellent interpersonal skills and the ability to work as part of a dedicated team.

5. Exceptional communication skills, with the ability to adapt communication styles as necessary.

6. An emotional resiliency to work with challenging and often distressing situations.

7. A current UK driving license and access to a vehicle for work purposes.
