Social Worker – Calderdale

Seven Social Care is looking for a qualified Social Worker to fill an exclusive opportunity specialising in the wellbeing of adults in Halifax.

The role:

Supporting adults throughout Calderdale who have a palliative diagnosis of 12 months or less.

Working with people at home, in the hospice and hospital discharges for end of life care – support to return home or to 24 hours care homes.

Close partnership working with our health colleagues and other organisations.

Supporting family and informal carers – carers assessments etc.

Following the fast track and continuing health care process.

Mental capacity assessments and best interest decisions where appropriate.

Completing Care Act assessments, trusted assessment and risk assessment and support planning.

Safeguarding casework.

Key Responsibilities:

Ensure that individuals remain at the heart of their work and empower vulnerable adults to make choices and to maintain their own autonomy wherever possible.

Ensure that the needs of family members, carers, groups and providers of care are understood and work with them towards ensuring that vulnerable adults receive the appropriate support (which is the least restrictive option) whilst recognising the need for intervention at times.

Ensure that vulnerable and/or disadvantaged people are supported appropriately; they will make decisions that are in accordance with relevant policy guidance. They will deploy the correct use of legislation aimed at maintaining the independence of vulnerable adults and assist them to maximising their own abilities and networks of support.

Applicant Requirements:

  • Qualifications: Degree approved by the Health and Care Professions Council (HCPC).
  • Experience: 2+ years working as a qualified Social Worker in the UK.
  • Compliance: Up to date HCPC registration and DBS.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37.5 hours p/w.
