Social Worker – Hospital Discharge – Adults

Seevn Social Care are seeking a dedicated and compassionate Social Worker, specialising in Adult Hospital Discharges, for a role based in Gloucester. This role provides a critical service in assisting patients, including those with complex needs, transition from the hospital to their homes or other supportive environments.

Responsibilities:

1. Collaborate with clinical teams, patients, and their families to prepare for safe hospital discharges.

2. Perform various assessments, including mobility, independence, mental capacity and overall risk to establish the level of support required post-discharge.

3. Organise and co-ordinate support and care packages ensuring it meets the patient’s needs.

4. Maintain regular communication with patients, family members and other relevant care professionals involved in patient’s care.

5. Build strong relationships with local community resources including residential homes and domiciliary care agencies.

6. Comply with all regulatory standards and principles of the Health and Care Professions Council (HCPC).

7. Maintain timely and accurate record keeping in compliance with policy and confidentiality guidelines.

Requirements:

1. Degree in Social Work (or equivalent).

2. Demonstrable experience as a Social Worker with focus on Hospital Discharges for adults.

3. Registration with the Social Work England (SWE).

4. Excellent communication skills with the ability to empathise and understand individual needs.

5. Proficient in record-keeping and report-writing, able to meet deadlines effectively.

6. A thorough understanding of safeguarding procedures and the ability to apply this effectively within the role.

7. Own transport, valid driver’s license and willingness to travel within the area.
