Social Worker – Hospital Discharge – Adults

Seven Social Care are seeking a dedicated Social Worker, specialising in adult hospital discharges, to join our multidisciplinary team in Lincolnshire. This is a wonderful chance to deliver vital social services in a thriving healthcare facility while nurturing your own development.

Key Responsibilities:

1. Develop and implement discharge plans for adults from hospitals, ensuring a smooth and appropriate transition back into the community.

2. Undertake robust assessments of adults, their families, and carers to identify their strengths, needs, and outcomes.

3. Coordinate care and support services while liaising closely with multidisciplinary teams to maximise client safety and welfare.

4. Keep up-to-date records, prepare reports and participate in team briefings and supervisions.

5. Advocate on behalf of service users to ensure their needs are met and rights are protected against discriminations.

6. Promote resilient, high-quality social networks around individuals and families.

7. Consult and cooperate with various community resources and services to secure required services for patients.

Qualification and Experience:

1. A degree of diploma in Social Work.

2. Current registration with the Health and Care Professions Council (HCPC).

3. Relevant experience in hospital discharge planning, adult social care or related field.

4. Excellent knowledge of relevant legislation and guidelines.

1. Strong communication and interpersonal skills.

2. Ability to problem-solve, negotiate and advocate effectively.

3. Tact, discretion and great teamwork skills.

4. Ability to respond swiftly to emergencies and make difficult decisions.
