Social Worker – RRT – Bexley Council

Seven Social Care is looking for a qualified Social Worker in the Rapid response team at Bexley

The role:

Rapid respond team is an integrated discharge team of Health and Social Care Professionals (Nurses, Therapist (Physio/ OT) and social care workers).

This team provide support to Bexley residents who are having a traumatic times or crisis points within their lives as opposed to the everyday stresses which we all experience.

The team provide support to prevent hospital admission (avoidance), relief carers stresses and enhanced wellbeing and safety. RRT support client at home as much as possible to maintain their safety and wellbeing.

The integrated team work from 8am to 8pm but the social care staff work only to shift pattern (9-5pm or 10-6pm) and we work one weekend every month (with Friday and Monday as a day off). Social care staff covers duty twice a week and the expectation is for duty staff to be based in the office to cover duty.

Social Care Staff work from home two days a week.
