Support Worker/Case Coordinator – Community Support Services

Seven Resourcing are looking for a Support Worker in the community support service to work in Penzance, Cornwall. This is a full time position and is paid at £13.41 per hour.

The role:

Under the leadership and management of the Team Leader, to work within a team of Social Workers and other Case Co-ordinators to provide a service to adults with social care needs; achieving high quality, timely and person-centred needs assessments of individuals, planning for and working to improve outcomes and increase/maintain the independence of vulnerable adults within a locality.

To contribute to providing high quality standards of practice, working to the Practice Quality Standards for Adult Social Care, taking accountability for ongoing development of own learning. 

Case Co-ordinators are required to work collaboratively with colleagues from different teams, disciplines and partner organisations in order to provide a seamless service to individuals/families/carers. They will help to protect vulnerable adults from harm or abuse and support people to optimise their choices in order to live as independently as possible. They will work with communities, individuals, families and friends to promote their personal strengths.

The Case Co-ordinator will ensure they acquire and improve their knowledge and skills and contribute to the learning and development of other members of their team.

As representatives of Care & Wellbeing, behaviour and conduct should be 

professional at all times, reflecting the value of the department and the Council.

Key Responsibilities:

  • Assess the potential to improve outcomes/lives gathering information from a range of sources, including adults, carers; and relevant others; other agencies, and other people within the person’s wider support network
  • Promote independence by offering information and advice, signposting to other agencies as appropriate, and prompting contact with social and community resources
  • Deliver high-quality person-centred assessments and reviews within a defined service area to ensure a consistent and timely response. 
  • Accurately record assessments, reviews and support plans, ensuring a copy is provided to the person concerned and/or their carer
  • Maintain case records, keeping them up to date so that they can provide concise and accurate information about circumstances and plans within the limits of data protection
  • Promote and facilitate effective communication with families, carers and individuals and other agencies to ensure the views and wishes of individuals and carers are given appropriate consideration and are central to all decision making
  • Seek advice from colleagues, other professionals, and line manager where this supports safe and effective decision-making
  • Maintain awareness of policy and procedures to safeguard adults at risk of abuse or neglect, alerting others to risks as appropriate
  • Manage less complex cases under the guidance of the Team Leader, or qualified practitioner and in liaison with other partner organisations to achieve positive outcomes
  • To develop an understanding of the Mental Capacity Act (2005) through appropriate training and resources.  
  • To routinely undertake mental capacity assessments and best interests decision making, seeking support and guidance for more complex decision, where necessary. 
  • To work with other team members and multi-disciplinary colleagues from other agencies to meet the needs of vulnerable adults, operating as an effective team member
  • Develop and maintain knowledge of social and departmental policy and work within the appropriate legislative context; ensuring the welfare of the adult is paramount at all times
  • To act within a framework which promotes independence, prevention, reablement and self-determination; facilitating effective communication with families, carers, individuals and other agencies
  • Promote assistive technologies, minor adaptations and equipment required to maintain/enhance/develop independence
  • Identify and consider a range of resources which may support the individual to meet their needs and achieve outcomes that are important to them. This will include identifying strengths and skills they can develop, and resources in their social and community networks
  • To actively promote self-learning and effectively contribute to appropriate supervision, appraisal and development opportunities
  • To act to protect adults in need of support services in line with legal requirements and ASC procedures and to ensure that all aspects of the councils safeguarding policies are observed.
  • To provide mentoring, support and guidance to new staff. 
  • Contribute to more complex information gathering for Section 42 enquiries
  • To attempt to resolve initial informal complaints and contribute to the resolution of formal complaints.
  • Ensure compliance and effective evidence gather that support maximising residents independence and support the key themes of the CQC Assurance Framework.
  • Identifying the most appropriate care and support which supports residents within the financial constraints of the directorate. Considering technology and equipment as a means to prevent, reduce and delay long term support

Applicant Requirements:

  • Qualifications: HCPC Registered
  • Experience: 2+ years working within a similar role. 
  • Compliance: Current Standard DBS check. 
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37 hours p/w.
